What information is collected and how it is processed
In order that you receive the best treatment and aftercare advice the following types of data are collected:
Personal Data such as your name, email address and telephone number. Data can be provided by you through the completion of a form on the website or via telephone or email.
Special Category Data will be collected when completing the new client questionnaire at your first appointment. This will include medical history, treatment details and financial information.
Your personal data will only be used for the purposes outlined in this Privacy Notice. Your data will not be shared with any third party without your express permission unless we are required to by law.
The conditions of my professional liability insurance require me to retain client records for ten years from date of treatment. Your information is stored both on a password protected computer and paper copies in a file kept in a secure place.
You have the right to request a copy of your records at any time. If you would like to do so, contact me and I will provide them either electronically or in hard copy for you.
If you are in any way unhappy with how data is collected and stored, you have the right to complain to the Information Commissioner’s Office (ICO), the UK supervisory authority for data protection issues (www.ico.org.uk). However, please get in touch with me first so that I can try and resolve any issues for you.
